Jax Ethan Formo

  Our little angel, Jax Ethan Formo passed gently in the arms of Jesus with his loving parents by his side on Aug. 18, 2021.  He was born on April 17, 2020.

  Jax was the son of Hyrum and Camille Formo.  He is survived by his loving sister, Maddie.

  His paternal grandparents are Robert and Julie Formo and great-grandparent Melissa Formo.  His maternal grandparents are Alfonso and Laura Lopez, great-grandparents Joe R. Estrada (Carmen) and the late Odelia Q. Estrada and Alfonso and Beva Lopez.  He is survived by many loving Tios and Tias along with cousins who loved him deeply. 

  During his time here on Earth, Jax touched many lives.  He gave us a lifetime of joy and happiness and was known to many as “Jax the Warrior.”  His life was a brief gift to us that will live on forever.   Jax enjoyed listening to Disney lullabies, watching his sister Maddie as she lovingly talked to her “baby brother”, and especially enjoyed his baths. 

  A Memorial Service will be held Friday, Aug. 27, 2021 at 10 a.m. at Living Word Chapel in Oracle with a luncheon following at the Dudleyville Park. 

  Griffith Mortuary is handling the arrangements.
